APPOINTMENTS:The office is open Monday through Thursday from 7:00 A.M. to Noon and 1:30 to 5:30 P.M. The business office is open Fridays 9:00 A.M. to 1:00 P.M. for financial, scheduling, and emergency calls. We prefer to see all our patients on an appointment basis and ask that you call in advance so we can reserve a time for you. We respect your valuable time and will do all that is possible to adhere to your scheduled appointment. Please respect our time by being on time for the appointment reserved for you and giving us 24 hours notice if you find it necessary to change your appointment. This allows us time to give your appointment to another patient in need of dental care. If when an emergency arises we will do our best to see you that day. So that we will be better able to assist you, please call early in the day. If you have to call after hours, you will be informed how to reach the doctor (or the doctor covering for him)YOUR FIRST APPOINTMENT:This appointment usually will consist of a thorough examination of the oral cavity, necessary x-rays, and a cleaning. Please have any current x-rays from your previous dentist forwarded in time for your first appointment. As part of our prevention-oriented dental philosophy, each patient will be instructed in proper oral hygiene techniques as often as necessary to keep their mouth in a state of good health. If other treatment needs to be completed after the examination, follow up appointments will be given as you departCHILDREN'S DENTISTRY:Our practice is family oriented because we love children. We know that a child's earliest experience in a dental office will form their attitude toward dentistry for the rest of their life. With this in mind, our office is equipped and the staff dedicated to making this first impression a happy and rewarding experience. We have found that children are more receptive to dental treatment in the morning when they're refreshed. Most small children do best to have their first introductory dental visit between age 2 and 3 at the same time as the parent's (or an older brother or sister who is already a good patient) regular cleaning appointment. That way we can acquaint them with our dental environment and assess their readiness for their own first appointmentCONTINUING CARE VISITS:We hope that you share in our belief that regular preventive dental care is a sound investment in your total health. It is much easier to take care of any small problems before they become big ones. We will provide you a reminder the month your continuing care appointment is due. The reminders may also be a combination of post cards, emails, text messages and phone calls. Please phone or email us for an appointment when you receive this notice. You may also make an advance appointment if you wishPATIENT REGISTRATION FORMPlease download our Patient History Form. You may fill it out when opened in Adobe Acrobat, then print it and bring to the office on you first visit.» Patient Registration Form. FINANCIAL INFORMATION:We are making every effort to hold down the cost of dental care. You can help by paying at the time of your visit. If your treatment requires several visits you will be given a fee estimate and definite financial arrangements can be made. Please make these arrangements in advance. Major Credit and Debit cards are also accepted for the convenience of our patients. Balances over 60 days are subject to an 18% A.P.R. interest charge per monthINSURANCE:In addition, as a service to those patients with dental insurance, we will accept payment from all non-PPO type insurance carriers. YOU will be responsible for ANY balance if your insurance does not pay for any reason within 45 days of your visit. It is YOUR responsibility to be familiar with your dental benefits. NO insurance company pays all dental costs; some companies pay fixed allowances and others pay a percentage of the fee charged. It is your responsibility to pay your co-payment or estimated portion at the time of your visit. We encourage you to discuss insurance questions with our business office staff.
