Office Visits

Welcome

We would like to welcome you as a patient to our office. Our quality-oriented practice performs comprehensive general and preventive dentistry, including bonding, crowns and bridges, dentures, implants, and children’s dentistry. Our staff includes hygienists, specially trained to treat your gum problems and clean your teeth, as well as educate you about the latest in proper preventive oral hygiene.

Relaxed Atmosphere

Treatment is done in a relaxed atmosphere with a genuine commitment to quality, total patient care, and reasonable, fair dental costs. Headphones and relaxing nitrous-oxide gas is available for your comfort.

Appointments

The office is open Monday through Thursday from 7:00 A.M. to Noon and 1:30 to 5:30 P.M. The business office is open Fridays from 9:00 A.M. to 1:00 P.M. for financial, scheduling, and emergency calls.

We prefer to see all our patients on an appointment basis and ask that you call in advance so we can reserve a time for you. We respect your valuable time and will do all that is possible to adhere to your scheduled appointment.

Please respect our time by being on time for the appointment reserved for you and giving us 24 hours’ notice if you find it necessary to change your appointment. This allows us time to give your appointment to another patient in need of dental care.

If an emergency arises, we will do our best to see you that day. So that we will be better able to assist you, please call early in the day. If you have to call after hours, you will be informed how to reach Dr. Hawkins (or the doctor that covers for him).

Your First Visit

This appointment usually will consist of a thorough examination of your mouth, necessary X-rays, and a cleaning. Please have any current X-rays from your previous dentist forwarded in time for your first appointment.

As part of our prevention-oriented dental philosophy, each patient will be instructed in proper oral hygiene techniques as often as necessary to keep the mouth in a state of good health. If other treatment needs to be completed after the examination, follow-up appointments will be given as you depart.

This initial visit involves a review of your medical history. When you share your medical history, be sure to provide complete, up-to-date information on your health. Inform our office if you have experienced a recent hospitalization or surgery, or recently been ill.

Also let us know the names of any medications you are taking — whether prescription or over-the-counter products — and the name of your physician.

Children’s Dentistry

Our practice is family oriented because we love children. We know that a child’s earliest experiences in a dental office will form his or her attitude toward dentistry for the rest of life.

With this in mind, our office is equipped and the staff dedicated to making this first impression a happy and rewarding experience. We have found that children are more receptive to dental treatment in the morning when they’re refreshed.

Most small children do best to have their introductory dental visit between age 2 and 3, at the same time as the parent’s regular cleaning appointment (or that of an older sibling who is already a good patient). That way we can acquaint your son or daughter with our dental environment and assess the youngster’s readiness for the first appointment.

Continuing Care Visits

We hope you share in our belief that regular preventive dental care is a sound investment in your total health. It is much easier to take care of any small problems before they become big ones.

We will provide you a reminder in the month your continuing care appointment is due. The reminders may also be a combination of postcards, emails, text messages, and phone calls.

Please phone or email us for an appointment when you receive this notice. You may also make an advance appointment if you wish. Regular checkup appointments typically take about 45 minutes. Patients are seen by appointment only.

We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately.

Patient Registration Form

Please download our Patient History Form. You may fill it out when opened in Adobe Acrobat, then print it and bring to the office on your first visit.

Thank you for your confidence in choosing our office, and we welcome you into our practice. If you have any questions, please do not hesitate to ask. We are here to serve you.

Our practice is continually being built on the referrals of our satisfied family of patients. We would consider it a pleasure to see one of your friends or relatives in need of dental care. Our best patients are referred by our best patients! Thank you for your referrals. We look forward to seeing you soon!
